How to Report the Birth of Your Child at the Philippine Embassy SG

Congratulations on your new baby! If you are a Filipino OFW and your baby was born here in Singapore, you will need to report the birth and get a passport for your child with the Philippine Embassy in Singapore.

This is an important step you need to make so that you can register your newborn and add him/her in our Phil. records.

Process of Registering the Birth of Your Newborn at the Phil Embassy in SG

First step in reporting the birth of your son/daughter is to send an e-mail to epassport.sgpe@gmail.com stating your request for an appointment and the baby’s full name.

Your personal appearance, together with your baby, will be accommodated between 1:00 pm to 4:00 pm only, Mondays to Fridays. Documentary requirements will include:

– Filled-up passport application form (click here to download)

– S$144.50 for passport (S$102.00) and notarization (S$42.50)

– Original and five (5) photocopies of the following documents:

o Photocopy of the data pages from the passport of both parents

o Report of Birth (click here to download the form, original signatures by the parent should be used)

o Birth Certificate of the baby from the Singapore Registration of Births

o Marriage contract of parents (if applicable)

If the parents are not legally married, please prepare the following additional documents:

– Affidavit issued by the father consenting to the use of his surname

– S$187.00 for passport (S$102.00) and notarization (S$42.50 x 2)

The parent can then apply for an NSO birth certificate for the baby after a minimum of six (6) months.
